Community support for young people's mental health
We use Evidence-based therapeutic interventions through RAID (Reinforce Appropriate, Implode Disruptive), PBS (Positive Behaviour Support) courses, psychological strategies in dealing with trauma and emotional dysregulation and the monitoring of psychotropic medication in the management of the young people we care for.
We support young people with complex presentations including “Looked After Children” and those subject to voluntary, Interim and Full Care Orders, as well as those with bail/remand conditions or Detention and Training Orders who require accommodation upon their release.
At Delicate Health Care we aim to work with Clinical Commissioning Groups (CCG)and Children Social Care Services using national best practice to improve the “step down” support and community provisions available for young people after they are well enough to move out of tier4 in-patent mental health and Learning Disability units.
Our step-down facilities seek to make a significant contribution to the Transforming Care Programme. It provides a move-on pathway for young people by adjusting their support levels until they are ready to make the transition to living in the community with floating support.
Short to long-term interventions leading to rehabilitation to the family home or foster care with continued support.
Longer-term stays for those where a return home is not a viable option.
This is a bespoke service completely tailored to our young peoples’ needs. This means that we support our young people to live at home and take full responsibility for every aspect of their care and support by matching them with a carer(s) who will provide on-going support for them. This would promote supporting people within their own home and/or to access community, receive the right care, in the right place at the right time by supporting their recovery in the community.
Our mission is to deliver a range of care and support to children and young people with learning disabilities and/or physical disabilities, aiming to promote their independence and connection with their local communities. Led by an experienced consultant psychiatrist and supported by highly trained staff, we specialize in providing support for young people with complex and challenging needs. We offer assistance to individuals with diagnoses such as autism and Asperger’s syndrome, Down’s syndrome, dementia, communication support needs, sensory and physical support needs (e.g., mobility support needs), and those dealing with drugs and alcohol abuse. Additionally, we provide support for individuals with dual diagnosis, which refers to those with learning disabilities who also have additional mental illness or behavioural difficulties.
